Credit cards companies share so much of your data - they even sell it. They have been doing this for so long that it's hard for them to change. Good luck getting an anonymous credit card, the only option are the pre-paids. These actually come with an agreement that you give them your name and address. You can make up your name, and your address too if your not going to have anything shipped. Get a mailbox - one of those ones at the gun shop where they distrust the government.
Pre-paid credit cards can be bought over the counter for cash from some supermarkets and drugstores. Was it RiteAid?
Update:
Snarfed.org has some great ideas on how to get an anonymous credit card.

American Express Pre-Paid Gift Card
As of October 2007, the American Express Pre-Paid Gift Card can be purchased at many places including 7-Eleven and Walgreens. This can be purchased off the shelf for cash in $25, $50, $100, $200 & $500 denominations. Contrary to the site and the card instructions, it DOES NOT require activation or any personal details. See http://www.americanexpress.com/giftcard for more information. There's even a secure website to check activity. The main issue with the card is that it can't be used at airline, hotel, car rental, telecom, or ATMs. It also can't be used outside of the U.S.
