There are many ways that identity theft can occur. It may not even be considered identity theft, but if your identifying or personal data (i.e a credit card) are used by someone else representing them as their own, identity theft has occured. People have been posing as others for years, but with the internet their is no face-to-face contact, making it easier for people to lie. The theif doen't even need to be in the same country! Private information is illegally sold every day by hackers that have found ways into "secure" systems. Credit cards, PayPal accounts, site logins, banks and brokerages are all exploitable - if money can be made, it's sure to be exploited. There have been many reports of companies and governments having entire customer databases compromised and exploited, while all the time we are reassured that it's "only" a minor breach.
